If you have a reliable source of sanity regain, you can use the Pile O' Balloons to make some inflated balloons. They will pop when hit and can cause chain reactions. Make some handy traps for pesky hound attacks! They can do surprisingly high amounts of damage.
Thinking outside the box is what will optimize the effects of the Pile O' Balloons. Not using it or misusing it is only making it harder to survive. (as if playing Wes wasn't making it hard enough...)
