The first noise for Pugna's voice during the beta was a drum. Then, Klei altered his voice several times, with a mix of drums and synth and organ, sometimes with just one instrument, sometimes many. Then Klei, settled on the noise it is today. A synth (sounding kind of like a deeper maxwell voice). Hope I answered your question.
